Replay catches pastes, drafts, and plan cards

What changed

• Session replay now shows a paste at the moment it happened, text typed into an Agents window chat box but never sent, and plan cards folded or open as the participant had them. It starts in the light or dark mode they used.
• Interview follow-up questions in a report are now written from every file the participant handed in. They no longer claim the code stops early or is missing a file it has.
• The submit dialog counts questions still on their starting code and says they score 0. The assessment terminal runs node, npm, and npx again.
