Candidate report with Score and Skills cards

What changed

• The candidate report now opens with a Score card and a Skills measured card. The explanations moved into tooltips, so the evidence comes first.
• On assessments that are not Agentic, the Questions table has a new Integrity column. It counts pastes, tab switches, and focus losses on each question, and each count links to that moment in the replay.
• The Integrity tab's details now sit at the bottom of the Evidence and replay tab. Time on each question shows active work next to time open, and the candidates table for an assessment is more compact.
